D.C. Provider Network, Amerigroup DC Take Value-Based Approach to Medicaid

The DC Connected (DC CCN) care network is a clinically integrated network that includes seven community health centers based in Washington, DC and the Primary Care Association of DC (DCPCA). His new collaboration with Amerigroup DC is the first agreement based on the value with a supplier of administered medical care in the capital of the nation.

Amerigroup DC is a total property subsidiary of Elevance Health. This three -year collaboration is the first of its kind in the district and is designed to transcend traditional health models, prioritize the results of patients and establish a new standard for the provision of medical care throughout the region.

The DC CCN consists of DCPCA and seven federally qualified health centers (FQHC): bread for the city, community of hope, family medical advice, the clinics of the town, Mary’s Center, Unity Health Care and Whitman-Walker Health. The mission of the network focuses on achieving a measurable impact through specific initiatives designed to reduce visits to the non -emerging, non -emerging emergency room (lane), reduce avoidable hospitalizations and minimize preventable hospital re -enters.

According to this new agreement, Amerigroup and DC CCN are changing the structure of medical care payments to rely on the quality and effectiveness of care instead of the volume of services provided. The agreement includes the commitment to share the financial risk, encourage FQHC to work in collaboration to achieve quality measures and improve health results for medicaid members.
